Certificate of Good Moral Character (CGMC)
A CGMC certifies that you have no pending obligations with the SAS Office or the University. It is required for the release of academic credentials, such as the Official Transcript of Records for graduates or transfer credentials for students transferring to another institution. It may also be required for scholarships, employment, or other legal purposes.
  1. Visit the SAS Office and complete the SAS Document Request Form.

  2. For fresh graduates, present your University Clearance signed by all relevant signatories except the SAS Campus Coordinator.

  3. Pay a fee of ₱20.00 at the Cashier’s Office.

  4. Submit the completed form and Official Receipt to the SAS staff for processing.

  5. Obtain signatures from the Student Discipline Unit Head and SAS Campus Coordinator.

  6. Proceed to the Office of the University Registrar for the certificate's dry seal.

The CGMC is valid for six (6) months from the date of issuance.
Yes, each copy costs ₱20.00.

Student Insurance and Mutual Aid
It provides financial assistance for hospitalization, accidents, or death.
Students driving vehicles must:
  • Hold a professional driver’s license.
  • Drive a registered vehicle.
  • Abide by LTO rules and regulations.
Parents or guardians must submit the following to the SAS Office:
  • A completed Student Mutual Aid Claim Form and Insurance Claim Form.
  • An official accident report or medical documentation, depending on the situation.
The processing time depends on the completeness of the submitted documents. The SAS Office will review the claim, and you will be notified once the disbursement is ready.
Reporting Violations or Filing Complaints
  1. If the grievance involves a student within your college:

    • Inform your Class Adviser to coordinate with the Guidance College Coordinator for referral to the Guidance Counselor (if counseling is needed).
    • File the complaint with the Office of the College Dean, which handles disciplinary actions.
  2. If the grievance involves a student from another college:

    • Inform your Class Adviser to coordinate with the Guidance College Coordinator for referral to the Guidance Counselor.
    • File the complaint with the SAS Office, where the Student Disciplinary Tribunal will manage the case.
  3. If the grievance involves a faculty member (academic issues):

    • Raise your concern with your Class Adviser for escalation to the Department Chairperson or College Dean.
  4. If the grievance involves a faculty or staff member (non-academic issues):

    • Inform your Class Adviser to coordinate with the Guidance College Coordinator for counseling, if needed.
    • File the complaint with the SAS Office, which will elevate the matter to the University Grievance Committee.
No. The University handles only cases that occur within its jurisdiction.
Anonymous complaints are not prioritized. Complainants may be required to attend hearings to provide testimony.
