• Hard copy of Transcript of Records (TOR) and Diploma

• Hard copy of Report of Rating (CS Sub-Prof/CS-Prof), if applicable

• Hard copy of PRC License and Board Rating (R.A. 1080), if applicable

• Service Record and/or Certificate of Employment (photocopy) from previous employer, if applicable

• Clearance and/or Certificate of No Pending Case (photocopy) from previous employer, if applicable

• SPMS/Performance Ratings or Performance Evaluation Certifications (photocopy) for the last two rating periods or semesters, whichever is applicable

• Training/Seminar/Course Certificates (photocopy), if any

• Designation orders/Initiative or output documents (photocopy), if any

• Certificates of outstanding accomplishments/awards (photocopy), if any

• Other relevant documents to support your application for the position
