Hard copy of PRC License and Board Rating (R.A. 1080), if applicable

Clearance and/or Certificate of No Pending Case (photocopy) from previous employer, if applicable
SPMS/Performance Ratings or Performance Evaluation Certifications (photocopy) for the last two rating periods or semesters, whichever is applicable
Training/Seminar/Course Certificates (photocopy), if any
Designation orders/Initiative or output documents (photocopy), if any

Both campuses—Bayombong and Bambang—have their respective HRMU and Records Unit offices. After verification, applicants shall submit the printed copy of their complete application documents to the Records Unit, located at the Administration Building of their respective campuses.
